The Gateway Supported Housing Program provides permanent housing for 46 individuals in Ulster County and 8 individuals in Dutchess County. The purpose of this program is to enhance the individual participant's options for choosing, accessing, and maintaining preferred long term housing, and provide support needed to maximize the recipient's successful integration into independent community living.
The program will rely on the participant's role in identifying their housing needs and actively pursuing the skills needed to succeed in their preferred housing.
The program intends to expand the variety and amount of affordable housing available to serious and persistently mentally ill persons and assist them in maintaining their residence in the community.
TARGET POPULATION
Individuals discharged from certified community residence programs at all levels and psychiatric centers as well as individuals that are currently homeless or living in such inferior housing that it puts them at high risk of hospitalization.

ELIGIBILITY REQUIREMENTS
Each applicant will be interviewed prior to admission to determine if the program is appropriate for meeting his/her needs.
No person shall be excluded from the program on the basis of race, religion, sex, or national origin.
