The Gateway Certified Apartment Program is a transitional apartment program. It provides opportunity for 27 mentally ill adults to live in a semi-independent setting while developing skills to fulfill their ultimate goal of independent living.  This program is certified by the State Office of Mental Health.  There is professional supervision from counseling staff at least two times weekly or as needed. 

 

The program includes a 5 unit (2 bedroom) complex in Kingston and eight 1-3 bedroom  scattered units in the city of Kingston. The 5 unit complex offers on-site staff 20-24 hours a day.  Residents in the scattered sites receive visits depending on individual support needs (2-5 times a week).  Residents are encouraged to assume as much responsibility for themselves as possible, as a further step toward independent living.

The Certified Apartment Program is a transitional residence with a 3 year average length of stay.

Eligibility Requirements: 
•   Persons age 18 or over

•   Primary diagnosis of mental illness

•   In need of supervised living, but possesses the ability to gain skills needed for more independent living

•   Each applicant will be interviewed prior to admission to determine if the program is appropriate for his/her needs

•   No person will be excluded from the program on the basis of race, religion, color, sex, or national origin
 

Services Provided:

•   Training & supervision in the activities of daily living

•   Counseling

•   Crisis management

•   Medication management

•   Socialization opportunities

•   Room and board

•   Transportation

•   Referral to and coordination with appropriate health, mental health, rehabilitation, and supportive services
 

Service Planning:

Each individual participates in developing his/her own Individual Service Plan, which identifies training and supervision needed to address functional limitations. Typically, individuals require assistance in developing:

•   Interpersonal skills

•   Employment potential
•   Ability to carry out activities to live independently
•   Nutrition and personal care skills
•   Cooperativeness, problem solving skills
•   Self-medication skills
•   Managing psychiatric symptoms
•   Counseling
   Crisis Management
   Medication Management
   Socialization Opportunities
   Room & Board
   Transportation


 

Fees: 

The fee is based on SSI Level II Residential Care and Medicaid Restorative Service rates for community residence and is subject to change.
