W. James Penrose Community Residence is a supervised home for 12 dually diagnosed (MH/MR) adults who have the ultimate goal of living independently. The residence is certified by the Office of Mental Health and has a 24-hour professional staff on duty.The residence is located on a semi-secluded property off Main Street in the Town of Rosendale. The three-story Victorian building offers rooms for group activities as well as single occupancy bedrooms. Residents have an excellent ability to gain practical knowledge of community living as they utilize local facilities which include a variety of shops, library, cinema, recreational park, churches, etc.

Service Planning:

Each individual participates in developing his/her own Individual Service Plan which identifies training and supervision needed to address functional limitations. Typically, individuals require assistance in developing:

Interpersonal skills
Employment potential
Ability to carry out activities to live independently
Nutrition and personal care skills
Cooperativeness, problem solving skills
Self-medication skills
Managing psychiatric symptoms

Services Provided:

Training & supervision in the activities of daily living
Counseling
Crisis management
Medication management
Socialization opportunities
Room and board
Transportation
Referral to and coordination with appropriate health, mental health, rehabilitation, and supportive services

Eligibility Requirements:

Persons 18 years or older
Primary diagnosis of mental illness with a secondary diagnosis of mild mental retardation, F.S.I.Q of 85 or less.
In need of supervised living, but possess the ability to gain skills needed for more independent living

Each applicant will be interviewed prior to admission to determine if the program is appropriate to meeting his/her needs.No person shall be excluded from the program on the basis of race, religion, color, sex or national origin.

Fees:
