Gateway's Continuing Day Treatment Program is designed to provide
individuals with psychiatric disabilities active treatment, support and skill
development in an empowering setting, which will enhance and promote success
and satisfaction in current, community based life roles and develop readiness
for choosing and attaining new life goals.

Services are provided Monday through Friday, 9:00
a.m. to 2:45 p.m., with most consumers attending at least three days per week.
Transportation may be arranged through Gateway Community Industries.
Gateway's Continuing Day Treatment services are Medicaid reimbursable;
private pay, sliding scale, and insurance may also be applied.
ADMISSION CRITERIA:
Individuals must:
- Be at least 18 years old
- Have a primary diagnosis of mental illness
- Be able to participate productively and safely in program activities

APPLICATION INSTRUCTIONS:
Referral sources may arrange for a screening by submitting the
following information to:
Ulster County Mental Health Adult Intake Unit

- Evidence of DSM IV Axis 1 Diagnosis of Mental Illness
- Psychiatric Evaluation
- Psychosocial Summary (Core History and Core Evaluation may
substitute)
- Medical History for the last six months, including meds and
expiration dates; Lab Slips, as appropriate
- Health or Physical Assessment, including evidence of TB screening
conducted within the last year
- Current Treatment Plan
- Discharge Summary from Current Treatment Provider
- Progress Notes for the last three months from Current Treatment
Provider
- Any other pertinent assessments
